The All-Care Mess

All-Care Notice - Nov. 21 2011

Dear All-Care patient's,

The day has finally arrived. We have secured your records. I know many of you are very anxious to get your records, so that you can find another dentist and continue on with your life. I assure you that we will get each and every one of you a copy of your records. With that said, my staff and I are going to be very busy in the coming weeks making these available to you. I would like to prioritize some people and would appreciate if you do not call us until we're ready to deal with your phase otherwise you will not allow us to serve the people that are in the most dire need. Here is how I would like to proceed:

Phase 1 - Emergency situations - pain or swelling present. These people can't wait. Hopefully this phase will be finished within the first day or so of this.

Phase 2 - Treatment that has begun and has not finished - examples are anyone that has had a denture, crown or root canal started and the treatment has not been finished.

Phase 3 - Next week - Anyone who has an appointment at a dental office within the next week

Phase 4 - Routine record transfer's - If you were previously seen at All-Care and do not fit into the above to classifications.

Phase 5 - Record transfer's for legal reasons - I would assume that some of you may be trying to get money back and that your attorney may be interested in viewing your records. From what I hear from Dr. Bates, there are no financial records available.

I will update this page to show what phase of this we are working on. Please reference this page to see if we've started your phase yet. If you would like to recieve your records, please fill out this form and fax it to us at 701-232-0203 or mail it to us when we begin your phase. Most of all, we're trying to help and are going to be overwhelmed, so please be patient with us.

We cannot be responsible for the accuracy or completeness of these records. They may or may not be complete. We are simply copying what was at All-Care, so that you can recieve a copy of them. We have become the custodian of these records; legally, we have to maintain the original copy.

We are planning to be open for longer hours.
